I'd like to thank everyone for their commentary so far. All of it is very constructive.
First, concerning the monsters from the ToH. I really wish everyone could own a copy of this book (so do the publishers!), but alas, there will be those who simply don't buy The Bonegarden because it depends so heavily on creatures from the ToH. It must depend on these creatures. Why? Because there simply aren't enough enough and other weird Bonegardenesque monsters in the MM.
In any future product I write for Necromancer, I will correct this problem either by using only the MM, or (more likely) including every pertinent bit of information on every monster not found in the MM.
Secondly, concerning the lack of update to 3.5 in certain places, the blame lies with . . . well, no one, really. We wanted to get the book in your hands as soon as possible. It was supposed to hit the shelves on Halloween of 2003. But we held off to update to 3.5. Alas, we rendered the updates with only galley copies of the 3.5 rules, and not the finalized books themselves. Put the blame on me. I was too anxious to see the module released!
Finally, the maps. Hmmm. Map quality is often a sticking point, isn't it? What I'll do is promise to provide the cartographers with a better quality map from my end, so that they have more to work with when it comes time to produce the final version. Deal?
