Hell, you can say 4e took a couple steps towards reality by adding the "Bloodied" condition, indicating some battle stress and a slight change in performance.
Not exactly realistic (D&D and hit points NEVER have been), but why not just tack on penalties to attack, defenses, whatever when a PC hits 3/4, 1/2 (bloodied), and 1/4 hit points? I think Gygax did something like this in Mythus/Dangerous Journeys (been a long time since I looked at those rules so I may be misremembering).
But- back on topic. Yes. Something must be done for sure about the "I rest 6 hours and am 100% better". I still dont know what the hell they were thinking when the penned that rule..but whatever. Easily house-ruled.
